Notice of Privacy Practices

Effective date: November 6, 2026

This notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully.

Our Commitment to Your Privacy

Rise Aesthetics, LLC (“Rise Aesthetics,” “we” or “us”) provides medical aesthetic treatments, and we treat the health information you share with us as confidential medical information. Rise Aesthetics is a cash-pay practice and does not bill health insurance. We have voluntarily adopted the privacy standards of the Health Insurance Portability and Accountability Act (HIPAA) and follow Arizona law on medical records.

This notice applies to your “protected health information” (“PHI”): information that identifies you and relates to your health, your treatment or payment for your care. This includes your health history, consultation notes, treatment records, consent forms and clinical photos.

We will follow the terms of this notice, keep your PHI private and secure, and tell you if a breach compromises the privacy or security of your PHI.

How We May Use and Share Your Health Information

  • Treatment. To evaluate you, plan and provide your treatment, and follow up afterward. For example, your provider reviews your health history and photos during your virtual consultation and before your treatment.

  • Practice operations. To run the practice and maintain quality and safety, such as training staff, reviewing outcomes and keeping records.
  • Appointment reminders and follow-up. To contact you about appointments, aftercare and treatment options by phone, text or email, as you’ve agreed.
  • Service providers. With companies that help us run the practice, such as our scheduling, intake forms, electronic health records, payment and secure messaging providers. They must protect your information and may use it only to provide services to us.

We may also share your PHI without your written permission when the law requires or allows it, including:

  • When required by federal, state or local law
  • For public health activities, such as reporting adverse reactions to products like fillers or neurotoxins to the FDA or manufacturers
  • To report suspected abuse, neglect or domestic violence to the proper authorities

Uses That Require Your Written Permission

We will not do the following without your written authorization:

  • Use your photos or videos in marketing, including our website, Instagram, TikTok or other social media
  • Use your PHI for marketing or to promote products or services outside your own care
  • Sell your PHI

You may cancel an authorization at any time by telling us in writing. Canceling does not undo anything we already did with your permission, but we will stop future uses. For example, we will not post your photos again and will remove them from our website where possible.
